Project:   Tagging Abnormal Vital Signs

Perceived/Reported problem:   abnormal vital signs, especially tachycardia and fever, have been too often inadequately reported or the assessment of them inadequately documented

Assessment: 
Discussion at the provider meeting revolved around developing a method of communicating this information in a way that would allow the nursing staff to be sure the provider saw the information of concern but would not require additional documentation by the nurse

Goal: 
assuring that abnormal vital signs are addressed by the provider and the basis of decision to accept variation from normal range at the time of disposition is documented

Start Date:  
12/5/2011                            Target Date for Completion:    1/26/2012

Milestones:  Posting of plan within 2 weeks
                             Assessment of usage at end of 1st month
                             Assessment of benefit at end of 2 months
