Project:   Initial Evaluation Standing Orders for the Bedside Nurse

Perceived/Reported problem:   The approach to patients between triage and provider care is very inconsistent.  Providers do not always trust nursing staff to be doing 'the right thing'.   Triage nurses are trying to do too much in Triage and tending to 'stick with' patients to assure good early care. 

Assessment: 
There are few standing orders, their format is inconsistent, they are not uniformly acknowledged or utilized.

Goal: 
To establish a well defined set of standing orders that will be accepted and utilized consistently by all providers so that nursing staff can have uniform expectations. 

Start Date: 
10/22/2011                               Target Date for Completion:    1/26/2012
                               
Milestones:   At the provider meeting on 1/26/2012 it was agreed to set up nursing staff training pending final approval from the providers.  The providers  have until next meeting to comment.   We will then be adjusting the set of protocols based on comments and as we gain experience with the set.  There will then be a 2 month trial with a goal to finalize the project at that time. 

Plan:  Protocols developed from industry standards, presented to the provider group for acceptance, then implemented via the Department Director.
